Physician/Dentist Patient Referral Form
Thank you for considering referring your patient to Dr. Miller. Dr. Miller works with both physicians and dentists to plan dental implant treatment.
Our Policy:
Since Dr. Miller is a general dentist, he wants you to know that it is his policy that anyone referred for placement of dental implants will NOT be seen for regular dental care. All patients will be referred back to the referring dentist.
Dr. Miller will work with the patient and with the referring dentist to plan the best possible result.
If you wish to refer a patient for evaluation, please print, fill out and either mail, send with patient or fax (415.453.1598) the form to Dr. Miller's office.
